Chestnut Hill takes an interdisciplinary approach when it comes to managing the resident’s short term or long term healthcare. Each resident is assessed by an experienced healthcare team who then develop a plan of care individualized to meet that resident’s needs. This approach ensures each resident is participating in a plan of care that will enable a smooth transition back to their routine activities of daily living.

Accommodations

Our Center features comfortable rooms that offer residents privacy and a quiet place to rest after a busy day of rehabilitation, socialization, and activities. Our sub-acute accommodations include free cable and flat screen televisions. Phone service is also available for a minimal cost.
